We have strong connections to Fort Memorial Hospital to ensure that your transition here is smooth. Our two in-house nurse practitioners have the expertise and compassion to provide excellent care that will restore your peace of mind as much as it heals your body.
Our fortified, in-house therapy department will help you regain your health, range of motion, balance, muscle tone and more. We offer physical, occupational and speech therapy up to seven days a week to those who need it, so that you don’t miss any of those critical recovery exercises. Our large gym has a wide range of equipment you can use, and ample space for everyone.
The last thing you want after a long stay at the hospital is to head right back. You can tell our subacute rehab program is right for you because we have such low 30-day readmission rates. The vast majority of our subacute patients leave our program to go home, where they stay. That’s because we offer post-discharge support and encouragement to keep you on track.
In May 2020, our average rate, specifically in our short-term rehab program, was roughly half the national average. In fact, in April, our 30-day readmission rate for the program was zero.
